There is a good introduction to this in Build Your Own Cat5 Cable Tester - Creating the circuit board. The main new gadget I got was a a Press-n-Peel Press from Techniks. Here are some pictures on how the NetAdminTools article has evolved with the new press:

Printing the film:

Scrub board with scruffy pad and Simple Green:

Place board in press at 300 degrees, with the iron preheated:

Leave in for 50 seconds, with the board clamped down pretty tight. If you leave the board in too long, the traces will bleed together. If you don't leave the board in long enough, the traces will peel off. I rinse the board off with cold water immediately after taking it out of the press. I then peel off the film.
